I’m are sorry to hear you are in pain, but appreciative that you have put your trust in me to get you back up and running as quickly as possible.

To help me gather the information I need to get you all set up and ready to see me. Please fill out as much as you can on this form by clicking here, and please give me a text message or email if you have any questions or need some help. Please note: the more information I have the easier it is to ensure you have the very best service and ensure your treatment is safe and appropriate.

Once I have your information about your reason to come and see me in my system, you will receive a text reminder of your appointment the day prior.

Some helpful hints for your appointment

  1. Please bring any medical records you might have regarding this condition with you
  2. Please arrive on time, as I want to ensure we can spend the full time allocated to help your condition
  3. Please wear, or bring to get changed into, appropriate clothing so we have access to the area to be treated
  4. Please let us know if there are any cultural or religious considerations that you would like us to be aware of prior to your appointment

Where to find us

Please ensure you have the right location when booking an appointment as I work from multiple sites. The maps for these locations are at the bottom of this page. 

Parking

CrossFit Taurus: enter the gate between Eastside Refrigeration and Kea Trailers. Car parks hare clearly marked upon entry. 

Car Park 3: these are CrossFit Taurus parks so use these if free

Car Park 1: only after 4pm

Car Park 2: only after 9am, not prior to that.

Car park 4 and Kea Trailers: NO PARKING!

Sport & Spine: There are 4 client car parks. Two are directly in front of Sport & Spine, and the other two are directly behind the building. There are clearly marked with the Sport and Spine logo. 

Redoaks Close: There is only one house on Redoaks Close. It’s at the end of the dead end street up a long and steep driveway. Just park at top of driveway and come on in. We have a small shihtzu maltese dog who loves to bark, and only bark. He thinks he’s a big dog haha. Just ignore him, I do.
