Dear valued New Patient

We are sorry to hear you are injured, but appreciative that you have put your trust in us to get you back up and running as quick as possible.

To help us gather the information we need to get you all set up and ready to see one of our team. Please fill out as much as you can on this form by clicking here, and please give us a call if you have any questions or need some help. Please note: the more information we have the easier it is to ensure you have the very best service and to gain ACC cover, if applicable, and ensure your treatment is safe and appropriate.

Once we have your information about your new injury in our system, you will receive a text reminder of your appointment at around 3:00pm the day prior.

Some helpful hints for your appointment

  1. Please bring any medical records you might have regarding this condition with you
  2. Please arrive on time, as we want to ensure we can spend the full time allocated to help your condition
  3. Please wear, or bring to get changed into, appropriate clothing so we have access to the area to be treated
  4. Please let us know if there are any cultural or religious considerations that you would like us to be aware of prior to your appointment

Parking

CBD Clinic: Our Rostrevor Street clinic has 3 clearly sign off-street parks directly in front of the clinic. Note: Please do not park in our neighbour’s parks (Baby on the Move) as cars will be asked to be moved. If these parks are taken, other free on-street parks can be found on Rostrevor Street & Vialou Street.

Melville Clinic: Our Urlich Avenue clinic either has free parking directly in front of the clinic, or if these are taken, there is a huge amount of free parking behind or scattered around the Urlich Avenue Shopping Centre.

COVID-19

At PhysioRx, we are standing with NZ in stamping out COVID-19. As such, we ask that you either scan the QR code with your NZ COVID Tracer App or sign in on arrival. We also ask that you inform staff if you are experiencing any cold or flu like symptoms, or have been in contact with anyone who has recently travelled overseas or has been diagnosed with COVID-19, prior to attending the clinic. We may ask you to not attend in person, but may be able to offer video conference/telehealth options in place of face to face. Our team can help guide you through this.
